SmarTone Plus members exclusive offers – Terms and conditions :
- The promotion period is valid from 28 November 2019 – 30 June 2020 (both dates inclusive) (“Promotion Period”).
- To enjoy the free Health Check Services (applicable to customer aged 18 or above) (“Promotion Offer”), the insured person (“New Customer”) is required to meet all of the following eligibility criteria:
a. Successfully enrols in Bupa MyBasic VHIS Plan, Bupa MyFlexi VHIS Plan or Bupa Hero VHIS Plan (“Eligible Scheme”) through a Bupa Health Management Consultant within the Promotion Period;
b. Must not cancel any individual medical insurance scheme underwritten by Bupa (Asia) Limited (“Bupa”) within 6 months prior to the date of submitting the application for an Eligible Scheme;
c. Has not enjoyed any child discount or family discount under the Eligible Scheme; and
d. Eligible Scheme must come into effect between 1 January 2020 and 1 July 2020 with annual premium payment paid.
- The redemption letters for the Promotion Offer will be sent to the New Customer by surface mail to the correspondence address in Bupa’s records within 2 months after the New Customer’s Policy Effective Date. New Customers can redeem the Promotion Offer at the designated service centres listed on the redemption letter within 1 year of the date of the redemption letter. No redemption letter will be re-sent by Bupa in case of loss or if the correspondence address on Bupa’s record is incorrect.
- In the event that the New Customer has redeemed the Promotion Offer and terminated the policy for any reason within 12 months after the Policy Effective Date, the New Customer agrees to repay Bupa the amount equivalent to the value of the Promotion Offer as an administration fee.
- Bupa is not the supplier of the Promotion Offer. The use of the service is bound by the terms and conditions issued by the service providers. Bupa makes no representation or guarantee as to the quality and suitability of the products, services or information provided by the service provider. Bupa shall not be liable for any matters arising from or in connection with the products, services or information related to the Health Check Service.
- The use and redemption of the Promotion Offer are subject to the terms and conditions set out in the redemption letters and imposed by the service providers.
- The Promotion Offer is only applicable to the eligible New Customer indicated in the redemption letters and it is not transferable to any other person or exchangeable for cash and other services.
- The usual retail prices of the Promotion Offer are provided by the service providers and are for reference only. Bupa is not responsible for setting these references.
- The Promotion Offer cannot be used in conjunction with other promotion offers.
- Bupa reserves the right to:
a. change these terms and conditions without prior notice;
b. make the final and binding decision in case of dispute in relation to this promotion; and
c. discontinue the promotion without prior notice.
- In the case of any discrepancy between the Chinese and English versions of these terms and conditions, the English version will prevail.
